New clients, please complete the appropriate child or adult intake form and the HIPAA privacy practices form prior to your first appointment. 

 

Existing clients, please complete a telehealth consent form if you have not already done so with your intake paperwork.

These are pdf files that can be printed, then scanned and emailed back to your clinician.  You can also complete them on your computer if you have Adobe.
